Community Partnership on Aging

The Community Partnership on Aging provides an open forum for providers and community members to discuss senior health and wellness. The committee meets the Fourth TUESDAY of each month from 9:00-10:30a.m., at the First Congregational Church, North Main Street, Wellsville, New York.

Objectives:

  • examine potential gaps in services
  • analyze cross-system collaboration opportunities
  • provide opportunities for senior fitness

GSAcounty

Senior Fitness is the key to Growing Stronger Allegany County. Based on Tuft’s University’s Growing Stronger curriculum, one-hour strength training sessions occur two times per week at ten sites across Allegany County.

ssteps

Senior Steps is a summer walking program for individuals, 50 years of age and older. Participants receive a free pedometer and tracking forms to monitor the number of steps taken on a daily basis throughout the summer months. Prizes are awarded at the end of the summer to everyone who participates in the program! Join us on May 28, 2008, Senior Health and Fitness Day, to begin your Senior Steps Walking Program.


PTfCaregivers

Powerful Tools for Caregivers is a six-week educational program, based on the Mather Life Ways curriculum, designed to provide caregivers the tools necessary to take care of themselves while caring for another. Classes will teach:

  • Stress reduction
  • Self-confidence
  • Communication skills
  • Decision making skills
  • Identify helpful resources


Trained facilitators from Allegany Arc, Allegany County Office for the Aging, Homecare and Hospice, Wellsville Manor Care Center, and Allegany/Western Steuben Rural Health Network, Inc. will instruct classes throughout the year. Please call to sign up for the next Powerful Tools for Caregivers program.

Seniors are invited to attend free quarterly educational workshops to increase knowledge and information on important health and wellness topics.


Iengagement

New Initiative- In addition to a healthy body, a healthy mind is also important as one ages. This new initiative will explore learning through a lifetime.

LTcareCouncil

The Community Partnership on Aging functions as the Long-Term Care Council of Allegany County’s Single Point of Entry system. The goal of the Single Point of Entry is to empower individuals to make informed choices and to streamline access to long-term care services. Long-term care services refers to the wide range of in-home, community-based, and institutional services and programs that are designed to help the elderly and individuals with disabilities of all ages.

New York State Office For the Aging and New York State Department Of Health envision an accessible, coordinated and person-centered long term care system that:

  • supports self-determination and promotes personal responsibility
  • provides services that meet consumer needs
  • provides quality care
  • ensures efficiency and affordability
