Sharing Your Wishes is an initiative begun in 2005, funded in part by the Community Health Foundation of Western and Central New York, and supported by the Allegany County Community Partnership on Aging and the Allegany / Western Steuben Rural Health Network, Inc.
The goal of this initiative is to assist the elderly in Allegany County and their caregivers so that they are better equipped to plan ahead for their health care during the last phase of life. The program offers a positive experience for completion of Advance Directives, encouraging reflection on current state of health, goals, values and preferences for future treatment decisions. Sharing Your Wishes provides a person with the opportunity to make end-of-life health care decisions known prior to experiencing a life-threatening illness or health-related crisis.
Studies show that up to fifty-percent of patients are incapable of participating in end-of-life decisions when the time comes. Does your family, physician and loved-ones know what your desires are, and would they support and respect your health care choices if asked to make end-of-life decisions for you? This question is the “heart” of the Advance Care Planning process.
